Premature Ejaculation (Early Discharge) Treatment in Delhi

Premature ejaculation (PE), also called early discharge, means you ejaculate sooner than you or your partner would like, and you find it hard to delay. It is one of the most common sexual problems in men. It can be treated, and many men improve once the cause is understood.

At Dr. P. K. Gupta Super Speciality Clinic in Karol Bagh, Delhi, Dr. P. K. Gupta (MBBS, MD, PGDS in Sexual Medicine) has more than 25 years of experience in sexual medicine. He first checks whether you truly have PE, then looks for the cause and builds a plan that fits your life and relationship.

Many men suffer in silence for years because of embarrassment. You do not need to. A private, honest conversation is the first step, and most men feel relief just from knowing their problem has a name and a treatment.

What Is Premature Ejaculation?

Doctors define PE by three things:

  • Ejaculation happens very quickly, usually within about one minute of penetration for lifelong PE
  • You have little or no control over delaying it
  • It causes distress, worry, frustration or avoidance of intimacy

All three matter. A short time alone is not enough for a diagnosis. If you and your partner are satisfied, there may be no problem to treat.

What is a typical time? Studies of men in many countries found the average time to ejaculation after penetration to be about 5 to 6 minutes. Times vary a lot between men and between occasions. Many men worry about being “too quick” when they are within the normal range, which is why an assessment helps.

Types of Premature Ejaculation

Doctors recognise four patterns. Knowing yours helps decide treatment.

  • 1. Lifelong PE. It has been present since your first sexual experiences, and it happens almost every time with every partner. It is thought to involve body factors, such as the way the nerves and brain chemicals control ejaculation.
  • 2. Acquired PE. You had normal control in the past, and PE started later. It is often linked to a health problem, stress, anxiety, relationship strain or an erection problem. Finding the trigger is key.
  • 3. Variable PE (natural variable). The problem comes and goes. It happens in some situations, such as after a long gap, with a new partner or when stressed. This is within the range of normal variation for many men.
  • 4. Subjective PE. You feel you ejaculate too fast, but your times are within the normal range. This is mainly a worry about performance, and it responds well to education and counselling.

Symptoms and Signs

  • You ejaculate before or very soon after penetration
  • You cannot delay ejaculation, even when you try
  • You feel anxious, embarrassed or frustrated afterwards
  • You avoid sex or new relationships because of it
  • Your partner is unhappy or confused
  • Worry about “finishing too soon” keeps your mind on it during sex

PE often brings anxiety, and anxiety can make PE worse. This cycle is one of the main things treatment helps break.

What Causes Premature Ejaculation?

PE rarely has one cause. Most men have a mix of body and mind factors.

Body-related causes

  • Brain chemistry. Serotonin, a brain chemical, helps control the ejaculation reflex. Differences in how it works are thought to play a part, especially in lifelong PE.
  • Sensitive penile nerves. Some men have a very sensitive glans.
  • Genetics. PE can run in families.
  • Hormone problems. Thyroid disorders and other hormone changes may play a role in some men.
  • Prostate or urinary infection or swelling. These can trigger acquired PE.
  • Erection problems. Men who worry about losing an erection may rush, which leads to early ejaculation.
  • Certain medicines or substance use. Some drugs and withdrawal from them can affect ejaculation.

Mind-related causes

  • Performance anxiety. Fear of failing keeps the body tense and speeds up ejaculation.
  • Stress and depression.
  • Hurried early experiences. Early sexual encounters that were rushed or secretive can set a pattern.
  • Relationship problems. Conflict, low trust and poor communication can add pressure.
  • Poor body image or low confidence.
  • Guilt or shame about sex.

Lifestyle factors

  • Heavy alcohol use or recreational drugs
  • Poor sleep and long periods of stress
  • Lack of exercise
  • Long gaps between sexual activity, which can make the reflex quicker

When Should You See a Doctor?

Book a consultation if:

  • You regularly ejaculate sooner than you want, and it troubles you
  • PE has begun recently when it was not a problem before
  • You also have erection problems, pain or burning when you urinate
  • You feel anxious, guilty or low because of it
  • It affects your relationship or you avoid sex
  • You have tried products or “delay” sprays and they did not help
  • You are unsure whether your timing is normal

If you feel hopeless or have thoughts of harming yourself, contact a doctor or a mental health helpline right away. Worry and low mood often go with this problem, and both can be treated.

How Is PE Diagnosed?

A careful assessment comes before any treatment. At Dr. P. K. Gupta Super Speciality Clinic, the visit usually covers these steps:

  • A private conversation. Dr. Gupta asks how long it takes, whether it has always been this way, how often it happens and how you and your partner feel.
  • Questionnaire. A short, standard questionnaire may be used to measure the problem and to track progress.
  • Health and medicine review. This covers medical conditions, medicines, alcohol and other substances.
  • Checking for other sexual problems. Erection difficulty often sits behind PE and is checked for.
  • Physical examination. This includes a general check and, if needed, a check of the genitals and prostate.
  • Tests only when needed. If there are urine symptoms or other signs, tests can look for infection, blood sugar or hormone problems.
  • Mood and stress check. Anxiety and low mood are common, and they are screened for.

Your partner can join the visit if you wish, but it is not required. At the end, Dr. Gupta explains what he found and which treatments suit you.

Premature Ejaculation Treatment in Delhi

Treatment depends on the type of PE, its cause, your health and your preferences. Many men use a mix of approaches. Dr. Gupta explains each option, its benefits and its side effects.

Behavioural techniques

These train your body and mind to delay ejaculation. They take practice but have no side effects.

  • Start–stop method. You stop stimulation just before the urge to ejaculate, wait for it to fade, then restart.
  • Squeeze method. Your partner gently squeezes the head of the penis for a short time to lower the urge before continuing.
  • Pelvic floor exercises. Strengthening these muscles can improve control. Some men benefit from physiotherapy guidance.
  • Slower breathing and relaxation. These help lower body tension during sex.

Counselling and sex therapy

When anxiety, stress or relationship issues play a part, talking therapy can make a big difference. Including your partner often helps. Therapy can also remove the fear of failure that fuels PE.

Topical products

Sprays and creams containing a mild numbing agent, such as lidocaine or prilocaine, are applied to the penis before sex. They reduce sensitivity and can delay ejaculation. They are used as directed, and some men find the area becomes too numb or their partner is affected. Condoms with a numbing agent are also available. Your doctor can advise on the right type.

Medicines

A doctor may prescribe medicines that raise serotonin levels and slow ejaculation.

  • Dapoxetine is made for on-demand use, taken a few hours before sex.
  • Daily medicines of the SSRI family, such as paroxetine or sertraline, are sometimes used and take some weeks to work.

These medicines need a prescription. Side effects can include nausea, dizziness, headache or a change in mood, and they are not suitable for everyone. Tell your doctor about other medicines, heart conditions and mood problems before starting. Do not buy PE tablets from a shop or website without a prescription, and do not stop or change a prescribed medicine on your own.

Treating related conditions

If an erection problem, an infection, a thyroid disorder or low mood is part of the picture, treating it can improve PE too.

What about surgery and injections?

Procedures that cut nerves or inject fillers into the penis are not standard care. Their evidence is limited and they carry risks. Dr. Gupta does not recommend them as a routine option.

Ayurvedic and herbal products

Many “delay” oils, powders and capsules are sold with big claims. Most have no solid proof, and some contain hidden drugs. Please check with a doctor before taking any.

Treatment progress is reviewed at follow-up visits, and the plan is adjusted. Many men improve with the right combination. No doctor can promise a permanent cure or a guaranteed time, but good control is a realistic goal for most men.

Healthy Habits That Support Treatment

  • Exercise regularly. Walking, jogging, swimming and strength training reduce stress and improve control.
  • Practise pelvic floor exercises as shown by your doctor or a physiotherapist.
  • Sleep seven to eight hours.
  • Limit alcohol and avoid recreational drugs.
  • Quit smoking.
  • Manage stress. Use breathing exercises, time outdoors and breaks from work.
  • Slow down. Allow time for foreplay and intimacy, and avoid rushing.
  • Talk with your partner. Honest conversation lowers pressure.
  • Avoid fixating on a timer. Watching the clock raises anxiety.

Common Myths About Premature Ejaculation

  • Myth: PE is only in the mind. Body factors, such as nerve sensitivity and serotonin, are often involved too.
  • Myth: It only affects young men. It can occur at any age.
  • Myth: It cannot be treated. Many men improve with the right treatment.
  • Myth: Masturbation causes PE. There is no proof it causes PE. Guilt and worry about it can add to anxiety.
  • Myth: Penis size is the cause. Size and PE are not linked.
  • Myth: Herbal oils and capsules fix it. Evidence is lacking, and some products are unsafe.
  • Myth: Taking an ED tablet cures PE. ED medicines treat erections. They help PE only when an erection problem is part of the cause.
  • Myth: Only the man is affected. PE affects both partners and the relationship.

What Happens If PE Is Left Untreated?

  • Anxiety and shame can grow
  • Confidence and self-esteem can drop
  • Intimacy and trust in the relationship may suffer
  • You may start to avoid sex or new relationships
  • A hidden cause, such as an infection or an erection problem, may go unnoticed
  • Planning a family can become harder, since ejaculation inside the vagina may not occur

Early treatment gives better and faster results.

How long should sex normally last?

Studies show the average time from penetration to ejaculation is about 5 to 6 minutes, and it varies a lot. If both partners are satisfied, there may be no problem to treat.

Can premature ejaculation be cured?

Many men improve a lot with treatment, and some are fully satisfied with their control. Because it can come back, doctors speak of good control, not a permanent cure. No honest doctor can promise a guaranteed time.
